Requirements of Family Care Coordinator / Case Manager -Bachelors in Social Work (or other related field) -Valid Driver's License and Car Insurance Benefits of Family Care Coordinator / Case Manager -Flexible Work Schedule -Excellent Supervised Support System -Company Phone -Mileage Compensation -401(k) -401(k) matching -Dental insurance -Employee assistance program Responsibilities of Family Care Coordinator / Case Manager:
-Establish initial contact with families within 24 hours of referral to assess safety, -Initiate safety planning, and connect families with resources to meet immediate needs; -Conduct team meetings and maintaining communication with the family and providers -Facilitating the implementation of each family's Action Plan -Make frequent and consistent contact with DCBS personnel -Provide direct services and education targeted at reducing risks, improving child well-being, and preserving child placement with the family per program guidelines.
