-Establish initial contact with families within 24 hours of referral to assess safety, -Initiate safety planning, and connect families with resources to meet immediate needs; -Conduct team meetings and maintaining communication with the family and providers -Facilitating the implementation of each family's Action Plan -Make frequent and consistent contact with DCBS personnel -Provide direct services and education targeted at reducing risks, improving child well-being, and preserving child placement with the family per program guidelines.
